My Way To Be Teen Video Challenge Official Rules

  1. Contest begins April 1, 2013. Contest ends no later than April 1, 2014, at the discretion of Maternal and Family Health Services. Video entries will be reviewed by a group of impartial judges from Maternal and Family Health Services, Inc. and the contest sponsors.  Random entries will be highlighted online (Safeteens website and social media outlets). No limit on the number of entries per person.
  2. Entrants must be legal U.S. residents, 13 to 21 years of age at time of entry. Employees of Maternal and Family Health Services (MFHS), its agents, affiliates, subsidiaries and members of their immediate families (parents, children) or other persons residing in the same household are not eligible.
  3. Monthly contests will run with designated themes. A Grand Prize winner will be chosen from the monthly winners. There is no entry fee. Monthly winners will be announced by the 15th day of the following month; Grand Prize winner will be announced no later than May 2014.
  4. Video max size: 2GB; Video Length: 15-30 seconds
  5. All entrants must complete a SUBMISSION FORM and Video Contest Consent & Release Form.  Visit and click on the “Enter Here” link. You will be directed to an email address where you will attach the video entry, submission form and Video Contest Consent & Release Form.  A signed MFHS Model Release Form must be submitted via fax, mail or, if signed and scanned, email at the time of entry for every recognizable person pictured in any video. If person(s) are under eighteen (18) years of age, a legal parent/guardian of the minor submitting the video and/or pictured must sign the MFHS Model Release Form of the minor.
  6. All entries must be submitted in digital format (compatible with YouTube.) Entries must be original, non-professional, neither previously published nor having won any other award. MFHS reserves the right at its sole discretion to reject and disqualify any nude, obscene, profane, offensive, lewd or pornographic subjects or photos, or any entry which MFHS, in its sole discretion, determines to be inconsistent with its standards, audience expectations or reputational interests.
  7. There is no limit to the number of entries submitted by any one person. You can submit as many videos as you want, but each video must be a completely separate and different video and your own originally created material.
  8. No copyrighted materials of any kind may be used for this contest unless you own the copyright or have a license to use the material for this contest. Written permission must be obtained and provided upon request for all copyrighted materials.
  9. MFHS assumes no responsibility for entries it is unable to process due to network, hardware or other technical failures or lost e-mail entries. MFHS reserves the right, in its sole discretion, to cancel Contest if it becomes technically corrupted or because of non-authorized human intervention. In the event of cancellation, MFHS reserves the right to award the prize to an eligible, non-suspect, entry received prior to date of cancellation. The use of automated entry devices or any other conduct which impedes the integrity of the Contest is prohibited. In the event of a dispute, entry will be deemed made by the holder of an established e-mail account associated with the entry.
  10. Posting of Entries: Select entries from the contest will be posted on as well as Safeteens social media outlets (Facebook, Twitter, Blogs, etc.) All decisions regarding the administration of the Contest shall be made in the sole discretion of MFHS and shall be binding and non-appealable. One winner named monthly and grand prize winners will receive the designated prizes in that category.
  11. Intellectual Property Rights: By entering the Contest, entrants agree to assign their intellectual property rights to MFHS and permit MFHS to use the videos in any media, including the Internet, for any purpose whatsoever. Entrants who do not agree to this use should not enter this Contest.   Required Release forms must be submitted for all entries or will be disqualified.
  12.  Judging: All submissions will be reviewed by a group of impartial judges from MFHS and our sponsors.
  13. Prizes:  Monthly winners will each receive a $25 Amazon gift card. Grand Prize winner in the Safeteens video contest will receive an Apple MacAir laptop.
  14. Entry and acceptance of prize constitutes permission for MFHS to use winner’s entry, name, hometown, likeness, photograph, voice and statements regarding this Contest for editorial, public relations, promotional and advertising purposes on behalf of MFHS without compensation unless prohibited by law. Grand Prize winner will be required to sign an Affidavit of Eligibility, Ownership and Release of Liability/Publicity, and Copyright Assignment unless prohibited by law, or forfeit winner status. If forfeited, another Grand Prize winner will be selected based on the next highest score. Subject to all U.S. federal, state and local laws and regulations. Void where prohibited. Applicable taxes are the sole responsibility of the winners.
  15. By participating, entrants release MFHS, its parent companies, affiliates, subsidiaries and agents, respective officers, directors, employees and agents from any and all liability or responsibility for any claim arising in connection with participation in the Contest or any prize awarded.
  16. By submitting a video, entrants agree to be bound by the official contest rules and decisions made by Maternal and Family Services, Inc. and the Safeteens Campaign.
